- Monthly Plan $99 - Includes 3 visits -3 procedures each visit. (Additional procedures $10 each) expires in one month.
- 10 Visit Plan $359 - 3 procedures each visit. (Additional procedures $10 each) expires in 6 months.
- 15 Visit Plan $499 - 3 procedures each visit. (Additional procedures $10 each) expires in 9 months.
- 20 Visit Plan $599 - 3 procedures each visit (additional procedures $10 each) expires in one year.
